This order form for our published books came home with your child yesterday. It is completely optional to order a copy of our book, but PLEASE return the form even if you’re not planning to order. Simply check the gray “NO” box at the bottom and return the form to school.

If you ordered online, please write the authorization code from your confirmation email on this form and return it to school.

In order to publish our book, we are required to have a returned form from each student.
